Paperwork / document fees
Paperwork that needs to be completed, such as emotional support letters, and letters for FMLA, disability, work, school, etc.

Accepted Payments
I accept payment via debit or credit card, Apple or Google pay. Payment is made prior to the appointment with the card on file, typically within 24 hours of the appointment time.

Aetna, Anthem Blue Cross, Blue Cross Blue Shield, Cigna, Oscar Health, Optum, Oxford, United Healthcare, UnitedHealthOne, GEHA, Tricare, and more.
Out of Network Billing
If you are out-of-network, I can provide you with a receipt/superbill to submit to your insurance for reimbursement. Most PPO plans will reimburse between 20-60% of the fee, but please check with your insurance.
